Dear Students,

 

To apply for your work permit, please have your students email Mr. Estuardo, KYDS ([email protected]). As a reminder, students must have a job in place for us to sign off on Work Permits.

To speed up the process, you can fill out the attached form ahead of appointment time. You need fill out this form completely with your parents signature and your employer signature. After you turn in this form in person, KYDS will print out the second form on site. Please feel free to contact KYDS if you have any question.
